That valve test at the Gulf refinery changed my pre-job routine

Was doing hydro testing outside Port Arthur last month and a 6 inch gate valve blew its packing at 150 psi, soaked the whole crew in water before we could isolate it. The inspector told me they'd seen three of these fail on the same manifold type in two years, all with the same torque spec. Now I double check the stem travel and packing gland tightness on every new valve install before we even hook up the pump. Any of you guys running into similar valve issues on older manifold setups?

The water hammer angle might be worth checking too, not just the torque on the packing. If the valve closes quick on that manifold, the shock can shift the stem just enough to wreck the seal over time. A slow close or a check valve upstream might save you more headaches than retightening those glands.

But honestly Ive seen way more packing failures from guys cranking down on the gland nuts way too hard than from any water hammer. You tighten those things like youre torquing lug nuts and the stem drags, then the valve wears uneven and you blame the system pressure. The real issue is most people never check the manufacturers spec for that packing torque. And if the water hammer was that bad, youd see pipe movement or the whole valve body cracking, not just a weepy seal. Plus adding a check valve or slow close device costs real money and downtime, while a simple re-torque takes five minutes and fixes it 90 percent of the time. Honestly feels like an overthinking engineer answer when the practical fix is right there.
