Did you check that you're using the right side of the bone folder? I made that mistake for weeks before someone showed me the edge matters way more than the pressure. Also, try scoring the fold line first with a dull tool before you burnish, it keeps the paper from stretching and messing up your hinges. Mine got way cleaner once I slowed down and did two light passes instead of one hard one.
