My plunge router bit walked off mid-cut on a maple door

I was doing a raised panel door in maple yesterday and the bit slipped about 3/8 inch during the profile pass, gouged the whole edge. Turns out I forgot to fully tighten the collet after swapping bits, and the vibration worked it loose over about 6 feet of cutting. Anyone else had a collet slip like that and found a better way to check tightness besides just cranking it down?

That's one way to learn why they call it a "raised panel" - because your blood pressure sure goes up when you see that gouge. Maple's not forgiving, that's for sure. I just give the collet an extra grunt now and do the twist-tighten-twist dance before every job. Cheaper than a new door.
