My $200 rain fly turned into a swimming pool in the Smokies
I dropped $200 on a fancy rain fly for a 4 day trip in the Smokies last month, and it held up great the first night. But on night two, water pooled on top and sagged right onto my sleeping bag because I didn't tighten the guy lines enough. Has anyone else had a rain fly fail from bad setup, or did I just get a dud?

30 years of backpacking and Ive seen more rain flies fail from setup than from bad gear. You basically described the classic mistake, too loose on the corners so the fabric droops instead of shedding water. My first tarp did the same thing in the Pisgah until an old timer showed me to pitch it drum tight with a steeper angle on the windward side. A $50 fly with good pitch beats a $200 one thats sagging every time.
