Got totally blindsided by a question in a Denver interview

I was in a final round interview for a project manager role at a tech startup in Denver last Tuesday. The hiring manager asked me point blank 'Tell me about a time you failed and what you'd do differently now.' I froze for a solid ten seconds because I'd prepped for everything but that. I mumbled something generic about a missed deadline, but I could see his interest fade. It completely threw off my whole vibe for the rest of the call. How do you guys prep for those curveball 'failure' questions without sounding like you're reading a script?
